ASHI Digital Certification Cards

Sold in sets of 5. ASHI Digital Certification Cards require a roster to be completed. Once your order is received, a fillable document for your roster will be emailed to you; complete document and email back to roster@ppmems.com. Instructor name or Training Site must be indicated on all orders. A receipt will be automatically sent to the email address provided during the order. PPM will assign certificates to students on your behalf once your order is processed and roster is received. You will recieve a copy of students' certificates for confirmation of delivery.
